About the Opportunity
The Field Hospital Enrollment Specialist serves as Pair Team’s on-site presence within hospital emergency departments and inpatient units. This role is responsible for identifying eligible Medi-Cal and Medicare patients, educating them about Enhanced Care Management, and supporting enrollment into Pair Team’s care navigation program before or shortly after discharge.
This is a patient-facing outreach role embedded within a fast-paced hospital environment. You will work closely with hospital social workers, case managers, nurses, registration teams, and other hospital staff to identify eligible patients and create a smooth enrollment experience. This role does not provide clinical care, conduct assessments, or perform ongoing care coordination. This role will be based on-site at one of Pair Team’s hospital partner locations in Fresno County, Stanislaus County, or Los Angeles County.
What You’ll Do
- Review daily patient census reports and partner with hospital teams to identify and prioritize eligible Medi-Cal and Medicare patients.
- Conduct in-person bedside outreach in emergency departments, observation units, triage areas, and inpatient units.
- Verify patient eligibility, including active coverage, health plan, and current care management enrollment status.
- Educate patients and families about Pair Team’s Enhanced Care Management program.
- Enroll eligible and consenting patients before discharge whenever possible.
- Document outreach attempts, patient status, attestations, and enrollment activity in Pair Team’s electronic medical record.
- Coordinate warm handoffs and communicate pending follow-up needs to Pair Team and hospital partners.
- Participate in shift huddles, department check-ins, and hospital workflow meetings as needed.
- Build strong working relationships with hospital registration, social work, case management, nursing, and unit leadership.
- Escalate census delays, access issues, low referral volume, and other workflow barriers to the appropriate Pair Team leader.
What You’ll Need
- Bilingual fluency in English and Spanish is required.
- 1+ year of experience in patient outreach, community health, enrollment, patient navigation, care coordination, or a related healthcare setting.
- Previous experience working in a hospital, emergency department, inpatient unit, or other clinical environment strongly preferred.
- Experience communicating effectively with patients from diverse cultural, socioeconomic, and health backgrounds.
- Ability to explain healthcare programs clearly, answer questions, and build trust with patients and families.
- Comfort approaching patients in person and working independently within a busy hospital environment.
- Strong organization, follow-through, and documentation skills.
- Proficiency with Google Workspace, spreadsheets, electronic medical records, and web-based platforms.
- A valid, active driver’s license and reliable transportation are required.
- Ability to consistently commute to the assigned hospital location.
