About the Role

The Medical Documentation Specialist (MDS) is responsible for delivering accurate, timely, and comprehensive clinical documentation that supports healthcare professionals in providing high-quality patient care. Working as an extension of healthcare professionals, the MDS documents patient encounters within Electronic Health Record (EHR) systems while ensuring compliance with organizational quality standards, provider preferences, and applicable healthcare regulations.
Medical Documentation Specialists leverage Commure's Ambient AI technology and documentation workflows to reduce providers' administrative burden, enabling providers to spend more time focusing on patient care.
This is an entry-level opportunity designed for recent graduates and early-career professionals. Through a structured onboarding program, comprehensive training, ongoing coaching, and continuous feedback, Medical Documentation Specialists develop expertise in clinical documentation, medical terminology, healthcare technology, provider workflows, and documentation best practices while maintaining the highest standards of quality, confidentiality, and operational excellence.

Training & Development

Every Medical Documentation Specialist begins their journey with Commure's structured Training and Development Program, designed specifically for recent graduates and early-career professionals. The program provides a strong foundation through a combination of instructor-led learning, hands-on practice, real-world simulations, regular assessments, continuous coaching, and personalized feedback, enabling participants to build the knowledge, skills, and confidence required to succeed in the role.
The program is designed to prepare participants for live documentation responsibilities while fostering professional growth, operational excellence, and a commitment to continuous learning. Upon successful completion of the training program, participants transition into the Medical Documentation Specialist role with ongoing coaching, mentorship, and quality support to further develop their expertise and advance their careers within Commure.

Role Purpose

The Medical Documentation Specialist is accountable for:

  • Delivering accurate, complete, and timely clinical documentation that reflects healthcare professionals' intent.

  • Supporting providers by reducing documentation burden and improving workflow efficiency.

  • Maintaining high standards of documentation quality, compliance, and confidentiality.

  • Learning and applying clinician-specific documentation preferences, specialty workflows, and documentation standards through structured training.

  • Continuously building knowledge and proficiency while contributing to operational excellence within Ambient Operations.

Key Responsibilities

Clinical Documentation

  • Accurately document patient encounters within Electronic Health Record (EHR) systems.

  • Capture patient history, review of systems, physical examination findings, assessments, diagnoses, treatment plans, procedures, medications, and other clinically relevant information.

  • Ensure documentation accurately reflects the clinician's observations, clinical assessments, and medical decision-making.

  • Review completed documentation for completeness, consistency, and accuracy prior to submission.

  • Complete documentation within established turnaround time and productivity expectations.

  • Apply learnings from onboarding, coaching sessions, and quality feedback to continuously improve documentation accuracy and efficiency.

Clinician Support

  • Provide documentation support for clinicians across multiple specialties including, but not limited to:

    • Primary Care

    • Internal Medicine

    • Oncology

    • Orthopedics

    • Rheumatology

    • Other specialties as assigned.

  • Learn clinician-specific workflows, documentation preferences, and specialty requirements through structured training and practical experience.

  • Adapt documentation style and workflow to individual clinician requirements.

  • Communicate professionally and effectively while supporting providers and collaborating with internal stakeholders.

Documentation Quality

  • Produce documentation that meets established organizational quality standards and clinician expectations.

  • Learn and apply specialty/provider-specific documentation guidelines and best practices.

  • Identify documentation inconsistencies and seek clarification through appropriate channels when required.

  • Incorporate feedback from quality reviews, coaching sessions, and audits into daily practice.

  • Participate in quality improvement initiatives and calibration activities.

Technology & Workflow Management

  • Utilize Electronic Health Record (EHR) systems and documentation platforms effectively.

  • Develop proficiency in Commure's Ambient AI documentation technology and associated workflow tools through structured training and guided practice.

  • Navigate multiple systems while maintaining productivity, accuracy, and quality.

  • Follow established operational workflows, standard operating procedures, and documentation processes.

  • Escalate workflow, technology, or system-related issues through appropriate channels.

Compliance & Information Security

  • Maintain compliance with HIPAA and all applicable patient privacy, confidentiality, and information security requirements.

  • Protect Protected Health Information (PHI) in accordance with organizational policies.

  • Adhere to Commure's compliance, security, and documentation standards.

  • Complete mandatory compliance and security training within required timelines.

Continuous Learning & Professional Development

  • Successfully complete onboarding, certification, and ongoing training programs.

  • Continuously develop knowledge of medical terminology, specialty-specific documentation, healthcare workflows, and emerging technologies.

  • Stay current with documentation standards, organizational processes, and workflow enhancements.

  • Demonstrate curiosity, adaptability, coachability, and a commitment to continuous learning and professional growth.

  • Actively participate in refresher trainings, coaching sessions, quality reviews, and knowledge-sharing initiatives.

Collaboration

  • Collaborate effectively with Operations, Quality, Training, Product, and other cross-functional teams.

  • Participate in coaching sessions, quality calibrations, workflow reviews, and operational improvement initiatives.

  • Seek guidance from trainers, mentors, and quality teams to continuously improve performance.

  • Escalate documentation, workflow, clinician, or operational concerns through appropriate channels.

  • Support initiatives that improve documentation quality, clinician satisfaction, and operational efficiency.
