From the original posting

We’re hiring a Behavioral Health Case Manager to help patients access, remain engaged in, and transition between the services they need. This role will support patients with complex behavioral health needs, including those moving between levels of care, while maintaining the flexibility to support outpatient psychiatry and therapy.

You will work closely with patients, clinicians, internal program teams, insurers, and external partners to remove barriers, coordinate services, and close the loop on referrals and transitions. You will not provide psychotherapy, diagnose patients, or make independent clinical or level-of-care decisions. You will be expected to recognize changes in risk or acuity and escalate promptly to the appropriate clinician.

About Talkiatry:

Talkiatry is transforming mental health care by making high-quality psychiatry more accessible, human, and sustainable for both patients and clinicians. Co-founded by a patient and a triple-board-certified psychiatrist, we were built to solve one of healthcare’s most urgent challenges: millions of people struggle to access mental health care, while clinicians face growing administrative burden and burnout. Through our technology-powered, in-network platform, we’re changing that, creating a better experience for patients and empowering clinicians to focus on delivering exceptional care.

Today, Talkiatry is the largest dedicated psychiatry practice in the U.S., with 750+ psychiatrists, 250+ therapists, and more than two million patient visits delivered nationwide. At our scale, every innovation has real impact. Every workflow we improve, every technology solution we build, and every patient experience we redesign helps thousands of clinicians provide better care to hundreds of thousands of patients. We’re not just growing quickly — we’re redefining how behavioral healthcare is delivered in America.

Joining Talkiatry means joining a mission-driven team that is solving complex problems at scale, driving meaningful change in healthcare, and building the future of mental health care with compassion, innovation, and purpose.

You Will:

  • Works with patients across new and existing behavioral health programs, adjusting priorities based on patient need, acuity, and program demand.

  • Support patients through enrollment, ongoing engagement, discharge, step-down, and return to outpatient care, with clear ownership of each transition until the next service is confirmed.

  • Conduct timely outreach, identify barriers to care, and help patients navigate benefits, transportation, community resources, specialty services, and other supports.

  • Coordinate closely with psychiatrists, psychiatric mental health nurse practitioners, therapists, primary care providers, and external treatment partners.

  • Identify changes in patient risk, engagement, or functioning and escalate clinical concerns according to established protocols.

  • Participate in crisis coordination by facilitating referrals to emergency, inpatient, residential, or other higher-acuity services under clinical direction.

  • Manage inbound and outbound referrals, including communication with insurers and partner organizations, transfer of necessary records, and confirmation that handoffs are completed.

  • Document case management activity and patient communication accurately and on time in the electronic health record to support continuity, quality reporting, and compliant billing.

  • Maintain current knowledge of internal programs and external resources and contribute to improving the team's service repository and referral pathways.

  • Track workload, referral status, barriers, outcomes, and care transitions, and use that information to identify operational gaps.

  • Help build and refine case management workflows as Talkiatry expands its services for patients with more complex care needs.

You Have:

  • A bachelor's degree in social work, psychology, counseling, public health, human services, or a related field.

  • At least three years of behavioral health case management, care coordination, discharge planning, or healthcare navigation experience.

  • Direct experience supporting higher-acuity behavioral health populations, including crisis care, discharge planning, structured treatment settings, or transitions between levels of care.

  • The ability to work independently, manage multiple active cases, prioritize time-sensitive needs, and follow work through to completion.

  • Strong judgment about when a situation requires escalation to a clinical review or immediate emergency workflows.

  • Experience working with multidisciplinary clinical teams and external healthcare or community partners.

  • Clear written and verbal communication skills and experience documenting work in an electronic health record or care management platform and experience using AI tools to increase efficiency.

  • Comfort working in a virtual environment and adapting as programs, workflows, and responsibilities evolve.

Licensure is not required. An LMSW, LSW, LPC, LMHC, CADC, CASAC, or other relevant credential may be considered, but this position does not provide psychotherapy or perform clinical services.

How You Work:

  • Patient centered. You listen carefully, communicate clearly, and help patients navigate difficult systems without losing ownership of the next step

  • Strong follow through. You track details, close loops, and deliver on commitments

  • Sound judgment. You understand the boundary between coordination and clinical decision-making and escalate concerns quickly

  • Comfort with change. You can work effectively while new programs and workflows are still being built

  • Team oriented. You share information, ask for clinical input when needed, and contribute to improving how the team operates
